职位描述
门诊药房：
1. 审核并解读门诊与急诊电子处方，必要时录入药房计算机系统。
2. 评估并解决实际存在或潜在的处方相关问题。
3. 按照门诊药房工作流程与任务（例如：药品退库、处理欠发药单）执行日常门诊发药工作。
4. 为患者提供关于处方药、疫苗及非处方药的用药咨询。
5. 参与药房日常任务（例如：麻醉药品清点、记录保存）。
6. 能够为患者及医务人员提供药物信息服务。
7. 管理并控制门诊诊室与急诊科存放的常备药品。
8. 负责部门至少一项质量指标的落实。
9. 在药房部门内发挥团队协作精神。
住院药房：
1. 建立新患者用药医嘱单并进行入院药物重整。
2. 识别并记录患者带入医院的自身药物。
3. 在参与护士每日交班/查房前，审阅患者每日病程记录及化验结果。
4. 审核所有新开立的医师医嘱，包括日间手术病例。
5. 评估并解决实际存在或潜在的处方相关问题。
6. 为住院患者调配药品至患者用药车。
7. 为出院患者进行床边发药并完成出院费用结算。
8. 参与药房日常任务（例如：麻醉药品记录管理；输液常备药品的每周补充）。
9. 积极参与药房活动并遵守优良药房工作规范。
10. 能够为患者及医务人员提供药物信息服务。
11. 管理并控制住院病区及手术室存放的常备药品。
12. 作为药房质量指标的一部分，每月进行住院患者抗生素使用情况分析。
13. 制定特定药物的配制与使用指南。
14. 每月向部门进行至少一次病例汇报。
15. 根据需要支援门诊药房工作。
16. 在药房部门内发挥团队协作精神。
其他职责：
1. 参与晚间及周末轮班。
2. 在需要时协助库存专员进行药品采购（订购、收货、上架）。
3. 调配并妥善维护疫苗库存。
4. 在药学技师协助下配制部门常备制剂。
5. 负责完成药房经理指派的其他专项工作。
任职资格:
• 经认证的药学专业院校毕业。
• 具备3-5年药房（社区或医院）工作经验。
• 持有执业药师资格证书。
